Transcript Requests
A personal transcript is available upon request. Transcripts list courses taken through the University of Minnesota School of Dentistry's Continuing Dental Education Program since July 1, 1985, and include the following information: 1) the course title, fees paid, date completed and amount of credits received for any given course, 2) the total number of courses taken, 3) the total amount of fees paid, 4) the total amount of credit hours accrued, 5) and the date and name of the state board or credentialing agency where your credits were sent.
